Why diaphragm flutter is often mistaken for hyperventilation – and how to tell them apartWaarom diafragma-flutter vaak voor hyperventilatie wordt aangezien – en hoe u het verschil herkent
Shortness of breath, a racing feeling in the chest, fast shallow breathing and anxiety: many people with diaphragmatic flutter (also called diaphragm flutter) recognise these complaints. They are also the classic picture of hyperventilation. Because hyperventilation is common and diaphragmatic flutter is rare, the flutter is easily overlooked and complaints are attributed to stress, anxiety or “over-breathing” – sometimes for years. This page explains both conditions side by side.
Diaphragmatic flutter is a rare movement disorder in which the diaphragm – the main breathing muscle – contracts involuntarily and rhythmically, usually several times per second. The contractions are often visible or palpable as pulsations in the upper abdomen, and are also known as diaphragmatic myoclonus, belly dancer’s dyskinesia or Leeuwenhoek’s disease. Read more on symptoms, causes and treatment of diaphragmatic flutter.
Hyperventilation means breathing more than the body needs. Too much carbon dioxide (CO₂) is breathed out, which lowers the CO₂ level in the blood (hypocapnia) and makes the blood more alkaline. This causes typical complaints such as tingling in the fingers or around the mouth, dizziness or light-headedness, chest tightness and a feeling of not getting enough air. When these episodes keep returning, often together with stress or anxiety, doctors speak of hyperventilation syndrome. The diagnosis is not always clear-cut: other physical causes of the complaints should be ruled out first.

Film your bare upper abdomen during an episode and show the video to your GP. Ask whether a referral to a neurologist or pulmonologist for diaphragm testing (fluoroscopy, ultrasound or EMG) makes sense. See videos of diaphragmatic flutter for examples of what it can look like.
It can contribute to it. The extra diaphragm contractions disturb normal breathing and can cause a feeling of breathlessness, which may lead to faster breathing. Treating only the hyperventilation then does not address the cause.
No. Diaphragm flutter is an involuntary movement disorder of the diaphragm muscle. Hyperventilation is a breathing pattern in which you breathe more than you need. The symptoms overlap, which is why they are easily confused.
Nee.
Only a doctor can tell. Rhythmic pulsations in the upper abdomen with their own steady rhythm are a reason to ask about diaphragmatic flutter. Fluoroscopy, ultrasound or EMG of the diaphragm can show the contractions.
Stress and deep breathing have been reported as triggers for episodes. That does not mean the condition is “just stress”: the contractions are involuntary and can be measured.
